In order to be seen at Middle Creek Dental, you must have an appointment. Our days are typically fully booked and we try very hard to ensure that we stay on time. As a courtesy to us and to our patients, we ask that you show up to your appointment 15 minutes ahead of time.
We understand that life can come at you fast so if for any reason you cannot make an appointment we ask that you let us know at least 48 hours ahead of time. If you cancel an appointment late or do not show up for your appointment, you may be subject to a missed appointment fee.
If you have dental insurance, we will be happy to help determine what is covered by your plan and what portion of the responsibility falls on you. In order to accurately assist you, we would ask that you submit any and all insurance information regarding your plan to us along with your New Patient Paperwork at least two days before your scheduled appointment. If your insurance changes, please let us know immediately. Our business team is ready and willing to help you understand what your insurance covers.
If you are diagnosed and need treatment, we can also provide you with a plan that details and estimate of how much of the payment you will be responsible for as well as what your insurance will cover.
Our professional care is provided to you, not to an insurance company. The insurance company is responsible to you for the plan that has been purchased by you or your employer. You are always responsible to the doctors for your payments. We will do our best to accommodate you with any questions or concerns that you may have regarding the payment process.
Payments are due within 20 days of the statement date. We gladly accept Discover, Visa, MasterCard and American Express.
We are always happy to schedule second-opinion consultations to discuss treatments that have been diagnosed to you by other doctors. It is important to note that all referral and second-opinion appointments will be preceded by a thorough examination before we provide any diagnosis. Make sure to request that all records and x-rays from your previous provider is requested to be sent to our office before your scheduled appointment. It is important to note that many offices require between one and two weeks to transfer records.
